Welcome to City & Hackney QI
In City & Hackney we have been working with the QI methodology for over five years now and we are passionate about using the tools it offers to improve services to our patients.
We see it as an important part of our overall approach. We have a well embedded monthly QI forum where projects are discussed and reviewed and we use QI data over time approaches in our DMT meetings. QI projects are closely linked in to management processes so that QI is starting to feel more like business as usual. A summary of our projects is below, and we have come to focus on some large key projects: patients being in hospital only when they need to be, a population health project that embraces the Trust’s Triple Aim ambition focusing on people with SMI making the transition back to primary care as well as a reboot of the violence and aggression reduction work through time to think. We have completed over 20 projects so far with varying scales but all providing important learning about quality.
We want to get the balance right between large scale directorate wide projects that have a broad reach and the smaller projects that are about local change and all staff feeling empowered to look for changes, implement and test.

In keeping with QI, we are always open to developing our structure and set up, therefore this guide will be updated on an annual basis to reflect our learning and any changes to how we use QI.

- Understanding that the services we work in are interconnected and complicated systems,
- Using quick testing and learning in order to develop our understanding of what might
help to improve the system (Plan, Do, Study,Act, or PDSA, cycles) - Having a robust system of measurement, using data over time, in order to know whether you are seeing improvement and
- Understanding and working with the human side of change.
